Terms & Conditions

About Us

The Nine Clinic Limited is a company providing Private GP and Wellness healthcare services.

The terms ‘we’, ‘us’ and ‘our’ include Dr Simmy Kaur, Dr Claire Bassilious and any other doctor or person employed or engaged by The Nine Clinic Limited.

All of our doctors are registered with the General Medical Council and are on the appropriate specialist register for the services they provide. All are subject to annual appraisal and adhere to the process of continuing medical education and revalidation as set out by the GMC.

All Doctors that provide services are responsible for compliance with all relevant regulatory and local requirements relating to medical liability and professional indemnity insurance.

We are all committed to safe and ethical prescribing and medicines management outlined in the GMCs ‘good practice in prescribing and managing medicines and devices’.

Services provided

We will provide private general practice services in accordance with these terms and conditions.

The services may include face to face, telephone, email and video consultations and home visits. We also supply courses of medicines and prescriptions where required, liaise closely with other health care workers and services, and provide specialist referrals as necessary.

We provide our Services during normal business hours which may vary. Please see the homepage on our website for current business hours.

We provide urgent same day and pre-bookable appointments with a Doctor, Monday to Friday.

We do not provide out of hours services unless prior agreement.

Appointments are often available at short notice, 5 days a week but this is not guaranteed.

Emergency healthcare services are not provided. (If a patient has a medical emergency, they should contact the emergency services immediately.)

Registration and identification verification

In order to access appointments as a new patient, patients will first be asked to provide a copy of a photographic form of identification (passport or driving licence). You will also be asked to verify your home address. If you are registering a child, you will need to provide evidence of parental responsibility (birth certificate or guardianship order).

You will also be asked to complete a registration questionnaire which includes information about your current health problem, previous medical history and current medication. You warrant and undertake to complete the registration questionnaire openly and honestly, revealing and disclosing all relevant information truthfully to the best of your knowledge.

Personal medical history

By registering and using our service, you are entering into a private GP/patient relationship and as such, we will ask you questions about your personal health known as your ‘medical history’.

Your medical history will be maintained by the doctor in accordance with our Records and Record Keeping, General Data Protection Regulation (GDPR) and Data Retention policies. We take the retention of patients’ medical records very seriously and will use the best possible data protection processes and procedures to ensure that your health information is kept confidential and stored securely.

Please see our privacy policy.

Appointments

You can book an appointment with us via our online booking platform, telephone and email address. You are encouraged to give us as much advance notice as possible for same day appointments.

We will confirm our acceptance of your appointment request by sending you an e-mail that confirms the appointment details. It is your responsibility to ensure that the patient details provided for the appointment are accurate and up-to-date.

We do not guarantee the availability of any particular Doctor at any particular time. We will do what we can to arrange a consultation with a Doctor as soon as reasonably practicable but do not guarantee to offer consultations within a particular time frame. Clinic appointments are limited to the time slot selected at the time of booking the appointment.

For home visit services, we are unable to provide a chaperone service for any examinations and as such we reserve the right to decline assessments that may be perceived to place our medical staff at risk. We do not therefore offer intimate examinations as part of our home visit services, although we may be able to advise on further appropriate management.

If you are under the age of 18, we may require you to be accompanied by a parent or guardian at the time of the appointment.
